질병관리청에서는 해외 예방접종 완료자 입국 증가에 따라 해외 예방접종 완료자에 대해 국내 접종자와 동일한 방역원칙을 적용하기 위해 해외 예방접종 완료자가 격리면제서를 발급받은 경우, ‘해외 예방접종 완료자 확인서’를 발급하고 있습니다.

1.발급대상 : 가. 해외 예방접종 완료 사유 격리면제서 소지자

                나. 국내, 외에서 자체 예방접종을 완료한 주한미군, 주한외교단 및 동반가족

2. 인정백신 : WHO 긴급승인 백신*에 한하여 인정

                 *아스트라제네카, 화이자, 모더나, 얀센, 시노팜, 시노백

3. 발급개시 : 2021. 10. 7.(목) 부터

4. 발급방법 : 해외 예방접종 증명서류*와 격리면제서**를 가까운 보건소에 방문하여 제출

                 (주한 외교단 및 동반가족은 국내, 외 예방접종 증명서류와 외교공한 제출)

*성명, 생년월일, 백신종류, 접종일, 접종기관명, 접종기관 또는 해당 국가 보건 당국의 직인 등을 포함한 문서화된 자료. 한글이

  영문이 아닌 경우, 한글 또 는 영문 번역본과 번역 인증서류(개인번역본은 공증기관 인증 필요)를 함께 제출해야 함

**격리면제서를 분실한 경우는 반드시 입국 당시 주소지 관할 보건소에 방문하여야 함

KDCA(Korea Disease Control and Prevention Agency) issues a 'certificate of confirmation for overseas vaccination'. Those who have been vaccinated abroad with a quarantine exemption.

1. Subject to : A. Holders of quarantine exemption certificate vaccinated overseas

                       B. US Forces Korea, diplomatic corps in Korea and accompanying families who have completed their own

                            vaccinations in Korea and abroad

2. Types of Vaccine : AstraZeneca, Pfizer, Moderna, Janssen, Synopharm, Sinovac

3. Start of issuance : 2021. Oct. 7.(Thu) ~

4. Issuance method : Submit proof of overseas vaccination* and quarantine exemption** documents to a nearby public health

                                   center

*Document must includes name, date of birth, type of vaccine, vaccination date, inoculation institution name or seal of the

  national health authorities. If it is not written in Korean or English, English translation and translation certificates(individual

  translation requires certification from a notary institution) must be submitted together.

**If the quarantine exemption is lost, you must visit the public health center where you are located at the time of entry.
